HCC Network

Context:

The Health Care Collaborative of Lafayette County (HCC Network) underwent tremendous growth in recent years, evolving from a rural health coalition into a Federally Qualified Health Center operating multiple clinic locations. This exponential growth brought new funding and patients but also presented challenges.

Challenge:

With this scale and complexity, HCC needed a strategic planning process to support alignment across its expanded organization. Their prior plan was dense and difficult to implement day-to-day. The new plan had to shed outdated strategies, delineate board and staff roles, and ultimately produce a simple, focused roadmap accessible to all staff.

Solution: 

Platform Civic Strategies designed a phased planning process engaging both board and staff:

  • The board identified priority strategic questions to address upfront. This allowed high-level engagement on the "big picture."

  • Staff reflected on progress and obstacles under the old plan to ground the new plan in organizational experience.

  • Staff developed focused goals, metrics and strategies tied directly to the board's identified priorities.

  • Across multiple planning sessions, the team created a tactical roadmap to advance each goal.

The resulting one-page strategic plan has clear goals, measurable targets and actionable strategies. It balances high-level direction from the board with detailed implementation by staff teams. This aligned process will drive meaningful progress and impact as HCC continues to evolve.
